 Mon-Fri  8a-4p   40 hrs/wk 

The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting

• Provide staff training and support on medical issues and techniques as needed
• Perform heath care assessments and develop written health care plans    
• Consult with program staff on specific medical interventions, techniques and documentation requirements.

Requirements

• Current Registered Nurse license with at least one year experience.   Some experience with people with developmental challenges preferred.
• Valid driver’s license & Insured Vehicle
