Position Summary
Rebuilding Together Metro Chicago is seeking a dedicated and compassionate Occupational Therapist to support our Safe at Home Program. In this role, the OT will conduct in-home evaluations, develop personalized scopes of work, and provide client education and training, all aimed at enhancing safety, accessibility, and independence. The OT will collaborate closely with the Safe at Home Program Manager and Construction Manager to ensure that all home modifications are tailored to each client’s needs and comply with grant requirements. This is an exciting opportunity to make a meaningful impact in the lives of older adults and individuals with disabilities by helping them live more safely and comfortably at home.
Primary Responsibilities
Conduct home-based assessments to evaluate client needs related to aging in place, safety, and accessibility focusing on prevention.
Develop individualized work scopes for home modifications.
Complete timely and compliant documentation in alignment with regulatory and grant guidelines.
Recommend and instruct clients on the use of adaptive equipment and techniques as well as home modifications.
Provide client-centered education and training to support safe and effective performance of activities important to the client including Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Advise and educate clients, family members, and caregivers on modifications and strategies for improved function, prevention, and safety in the home environment.
Coordinate with program and construction staff to ensure modifications are implemented effectively and within grant and program parameters.
Conduct follow-up visits to assess the effectiveness of recommendations and modifications.
Assist in project setup and closeout, including obtaining documentation and client sign-off.
Track and maintain compliance with grant and program timelines and deliverables.
Collaborate with COO to train and mentor Occupational Therapist Capstone Student
Support RTMC’s mission through collaborative participation and attendance at program events, fundraising events, and initiatives as needed.
Compensation and Benefits
The salary for this position is commensurate with experience and falls within the range of $65,000 to $75,000 annually.

Requirements
Qualifications
Licensed Occupational Therapist in the State of IL (Required)
Expertise in evaluating needed home modification repairs including installing grab bars, handrails, and ADA equipment.
Mission-driven attitude with a passion for non-profit, volunteer, and social services.
Strong written, verbal, and interpersonal skills.
Superb analytical, organizational, communication, and problem-solving skills.
Ability to successfully multi-task and manage various projects and deadlines.
Familiarity with Google Documents and MS Office suite.
Creative, compassionate, self-starter with the ability to work both independently and as part of a team.
Must have a valid driver’s license, reliable transportation, and proof of auto insurance
Preferred
Bilingual (English/Spanish).
Two years of work experience with senior and disabled clients in the home health or residential setting.
Familiarity with Salesforce
Working Conditions and Environment/Physical Requirements
Physical stamina and agility are required.
Must be able to lift and/or move objects up to 25 pounds.
Must own a reliable, insured vehicle and have a valid driver’s license.
Travel between the main office and other locations will be required.
Project locations may not be accessible per the Americans with Disabilities Act (ADA).
Attend and participate in mandatory training, staff meetings, and other events as scheduled.
Occasional extended workday and weekend work are required.
